Akut ve Kronik İnmeli Hastalarda Uyku Kalitesi: Yaşam Kalitesi, Klinik ve Fonksiyonel Durum ile İlişkisi
Sleep Quality in Patients with Acute and Chronic Stroke: Its Relation with the Quality of Life, Clinical and Functional Status

ÖZET
Amaç: Akut ve kronik inmeli hastalarda uyku kalitesini, hastaların yaşam kalitesi, klinik ve fonksiyonel durumu ile ilişkisini incelemek.

Yöntemler: Çalışmaya, akut (n:13) ve kronik (n:17) inmeli 30 hasta ile yaşları ve cinsiyetleri eşleştirilmiş 30 kontrol dahil edildi. Hastalarda motor iyileşme Brunnstrom İyileşme Evresi (BİE), bilişsel durum Standardize Mini Mental Test (SMMT), psikiyatrik durum için yarı yapılandırılmış klinik görüşme ölçeği (SCID-I), fonksiyonel durum Fonksiyonel Bağımsızlık Ölçümü (FBÖ) ve yaşam kalitesi Short Form-36 (SF-36) ile değerlendirildi. Grupların uyku kalitesi Pittsburgh Uyku Kalitesi İndeksi (PUKİ) ile değerlendirildi.

Bulgular: PUKİ total skoru, akut (7.46±4.07, P=0.006) kronik (7.06±3.93, P=0.013) ve tüm inmeli hastalarda (7.23±3.92, P=0.001) kontrollerden (4.10±3.18) daha yüksekti. Akut dönemde uyku süresi (P=0.015) ve uyku bozukluğu (P=0.002), kronik dönemde subjektif uyku kalitesi (P=0.015) ve alışılmış uyku etkinliği (P=0.010) skorları kontrollerden yüksekti. Ancak akut ve kronik inmeli hastaların PUKİ total ve alt skorları arasında anlamlı fark yoktu (p>0.05). Yalnızca, uyku bozukluğu skoru akut hastalarda, kronik hastalardan daha yüksekti (1.76±0.59, 1.29±.058, P=0.038) . Akut inmeli hastaların PUKİ total skoru ile BİE ve FBÖ arasında anlamlı ilişki saptanmaz iken, yalnızca SF-36'ya ait ağrı skoru ile korelasyon saptandı (r=0.76 ve P=0.002). Kronik dönemdeki hastaların PUKİ total skoru ile kullandıkları ilaç sayısı (r=0.49, P=0.04), üst ekstremite BİE (r=-0.50, P=0.04) ve SF-36'ya ait genel sağlık (r=-0.52, P=0.03) skorları arasında anlamlı ilişki vardı.

Sonuçlar: Akut ve kronik inmeli hastaların uyku kalitesi sağlıklılardan daha kötüydü. Akut dönemde uyku kalitesi SF-36'nın ağrı skoru ile, kronik dönemde kullanılan ilaç sayısı, BİE ve SF-36'nın genel sağlık skoru ile ilişkiliydi.

ABSTRACT
Objective: To investigate sleep quality of the patients with acute and chronic stroke and its relationship with quality of life, clinical and functional status.

Methods: Thirty patients with 13 acute and 17 chronic stroke and 30 healthy controls (HCs) were included into this study. Motor recovery of the patients with Brunnstrom's Recovery Stages (BRS), mental status with Standardized Mini Mental State Examination (SMMSE), mood status with Structured Clinical Interview for DSM-IV (SCID-I), functional status with Functional Independence Measure (FIM) and quality of life with Short Form-36 (SF-36) were evaluated. The sleep quality of both groups was measured with Pittsburgh Sleep Quality Index (PSQI).

Results: The PSQI global score of acute (7.46±4.07, P=0.006), chronic (7.06±3.93, P=0.013) and all stroke (7.23±3.92, P=0.001) were higher than HCs (4.10±3.18). Sleep duration (P=0.015) and sleep disturbance (P=0.002) in acute stroke, subjective sleep quality (P=0.015) and habitual sleep efficiency (P=0.010) in chronic stroke were higher than HCs. There wasn't significant difference for PSQI global score and subscores between acute and chronic patients (p>0.05). Solely, sleep disturbance of acute patients was higher than chronic patients (1.76±0.59, 1.29±.058, P=0.038). While there wasn't correlation between the PSQI global scores of acute patients with BRS and FIM, there was a significant correlation with pain score of SF-36 (r=0.76 and P=0.002). There were significant correlations between the PSQI global scores of chronic patients with the number of used drugs (r=0.49, P=0.04), BRS of upper extremity (r=-0.50, P=0.04) and general health score of SF-36 (r=-0.52, P=0.03).

Conclusion: The sleep quality was poorer in acute and chronic stroke than HCs. It was associated with pain score of SF-36 at acute stroke, number of drugs, BRS and general health score of SF-36 at chronic stroke.
