DİYABETES MELLİTUSLU HASTALARDA HAREKET HIZLARI
MOVEMENT SPEEDS OF PATIENTS WITH DIABETES MELLITUS

ÖZET
Amaç: Diyabetes Mellitus (DM); vücutta insülin hormonu yetersizliği sonucu oluşan kronik metabolik bir hastalıktır. Seyrinde birçok komplikasyon gelişebilir. Bunlardan bazıları kas-iskelet ve sinir sistemini etkileyebilir. Bu çalışmada DM'un hastaların hareket hızlarında herhangi bir etki yaratıp yaratmadığının araştırılması amaçlanmıştır.

Yöntem-gereçler: Çalışmaya nörolojik muayenesi normal olan ve alt ekstremitede kas iskelet sistem şikayeti olmayan 26 DM'li hasta ve benzer yaş ortalamasına sahip DM'si olmayan 25 kontrol vakası alınmıştır. Tüm vakaların demografik verilerinin yanı sıra 65 m mesafeyi yürüme süresi ve adım sayısı, 9 basamak merdiveni inipçıkma süresi, sandalyeden kalkma ve çömelip doğrulma süreleri, bel çevre ölçümü, vücut kitle indeksi (VKI) ve yürüme süresi ve adım sayısı, merdiven inip-çıkma süresi, sandalyeden kalkma ve çömelip doğrulma süreleri, biyokimyasal değerleri değerlendirilmiştir.

Bulgular: Yürüme (hasta 59,1 sn ve kontrol 53,8 sn), merdiven inip-çıkma (hasta 7,8 sn ve 8 sn, kontrol 5,9 sn ve 6,5 sn), çömelip doğrulma süreleri; (hasta 4 sn ve kontrol 2,8 sn) diyabetik hastalarda kontrol grubuna göre daha uzun olup aralarındaki fark istatistiksel olarak anlamlıydı (p=0,017, p=0,004, p=0,006, p=0,001). HDL kolesterol düzeyi diyabetik hastalarda kontrol grubuna göre istatistiksel olarak anlamlı derecede düşük olup (hasta 40,1 mg/dl ve kontrol 49,6 mg/dl, p=0,003), her iki grubun hareket süreleri ve yürüme adım sayısı ile kan şekeri düzeyi, bel çevresi, VKI değerleri arasındaki ilişkiye bakıldığında bel çevresi-adım sayısı ilişkisi dışında süreler ve adım sayısı ile bu parametreler arasında istatistiksel olarak anlamlı bir ilişki olduğu (p=0,000) gözlenmiştir.

Sonuçlar: DM'nin; hastaların nörolojik muayeneleri normal olsa bile hareket hızlarında yavaşlamaya neden olduğu düşünülmüştür.

ABSTRACT
Aims: DM is a chronic metabolic disease due to insufficiency of secretion of insulin in the body. A lot of complications can develop during the disease, some of them affecting the musculo-skeletal system and the nervous system. In this study it is our purpose to investigate if DM could cause any effect on the speed of movement of the patients.

Material-Methods: This study included 26 DM patients and 25 control patients with similar age profile whose neurological examination was normal and had no musculo- skeletal system complaint in the lower extremity. Gait velocity and the number of steps during 65 m walk., period of climbing 9 stairs up and down, chair stand time and squatting and standing up time, waist circumference, body mass index (BMI), blood biochemistry of all patients were evaluated besides their demographical data.

Findings: Gait velocity (patients 59.1 sec. and controls 53.8 sec.), period of climbing 9 stairs up and down respectively (patients 8 sec. and 7.8 sec, controls 6.5sec. and 5.9 sec.) and squatting and standing up time (patients 4 sec. and controls 2.8 sec.) were longer than the control patients and this was statistically significant (p=0.017, p=0.004, p=0.006, p=0.001). HDL cholesterol levels in diabetic patients were statistically significant lower than the controls (patients 40.1 mg/dl and controls 49.6, p=0.003), in both groups when the relation between velocity of movements and number of steps with fasting blood levels, waist circumference and BMI measurements examined expect the waist circumference-number of steps measurements all the parameters showed statistically significant relationship with number of steps (p=0.000).

Results: It is considered that even if the patients have a normal neurological examination, DM causes a slowdown in the speed of movements.
