THE TREATMENT OF REFLEX SYMPATHETIC DYSTROPHY SYNDROME IN HEMIPLEGIC PATIENTS

ÖZET
Refleks Sempatik Distrofi Sendromu (RSDS), hemiplejik hastalarda rehabilitasyon programını etkileyen önemli komplikasyonlardan biridir. Bu çalışmada amacımız, hemiplejide geliÅŸen RSDS tedavisinde kalsitonin tedavisi ve fizik tedavi programının etkinliÄŸini karşılaÅŸtırmaktı. Serebrovasküler olay sonucu geliÅŸen hemiplejili ve RSDS olan 40 hasta çalışmaya alınarak 2 gruba ayrıldı. Birinci gruba günde 100 IU salmon kalsitonini intramusküler olarak 1 ay verilirken, diÄŸer grup fizik tedavi ( TENS, kontrast banyo 20 seans) programına alındı. Omuz El Sendrom Skoru (SHSS), tedavi öncesinde ve sonrasında iki grup için de RSDS'nun ÅŸiddetini deÄŸerlendirmek için kullanıldı. Birinci ayın sonunda SHSS'ndaki azalma kalsitonin grubunda, fizik tedavi grubuna göre daha anlamlıydı(p<0.001). “Motor Assessment Scale” (MAS) ile deÄŸerlendirilen üst ekstremite motor fonksiyonunda, kalsitonin grubunda birinci ayın sonunda anlamlı ilerleme gözlendi (p<0.05). Sonuç olarak bu çalışma hemiplejik hastalarda geliÅŸen RSDS tedavisinde kalsitoninin etkin bir tedavi seçeneÄŸi olduÄŸunu destekledi.
ABSTRACT
Reflex Sympathetic Dystrophy Syndrome (RSDS) is one of the important complications effecting the rehabilitation programs of hemiplegic patients. In this study, our aim was to compare the effects of salmon calcitonin and physical therapy programs in RSDS due to hemiplegia. Forty patients with hemiplegia resulting from cerebrovascular events and developed RSDS were included in the study and divided into two groups. The first group received 100 IU/day of salmon calcitonin intramuscularly for one month whereas the second group received physical treatment (TENS, contrast bath for 20 sessions). Shoulder Hand Syndrome Score (SHSS) was used to evaluate the severity of RSDS before and after treatment in two groups. At the end of the first month of treatment, the amount of decrease in SHSS of calcitonin group was more significant than that of the physical treatment group(p<0.001). There was significant improvement in motor function of upper extremity that was evaluated with “Motor Assessment Scale” (MAS) in the calcitonin group after first month of treatment(p<0.05). Briefly, this study supported that calcitonin is an effective treatment choice in RSDS that develops in hemiplegia.
