COST ANALYSIS OF PATIENTS WITH STROKE DURING THEIR STAY IN A REHABILITATION CENTER

ÖZET
Bu çalışmada rehabilitasyon merkezinde yatan inmeli hastaların ekonomik maliyetleri değerlendirildi. 2000-2004 yılları arasında rehabilitasyon merkezine kabul edilen 465 inmeli hasta değerlendirildi. Hastaların rehabilitasyonu sırasındaki ekonomik maliyetlerini tanımlamak için hasta dosyaları ve taburcuğundaki faturaları kullanıldı. Tüm hastalar için toplam yatış maliyeti hesaplandı. Total yatış maliyeti; Rehabilitasyon maliyetleri (tüm rehabilitatif ve medikal uygulamalar ile oluşan komplikasyonlara yönelik yapılan uygulamalar ve bunların harcamaları), laboratuvar maliyetleri, hastane maliyetleri şeklinde altgruplara bölündü. Yaş ve cinsiyetin maliyet üzerine etkisi yoktu. Rehabilitayon merkezine geç başvuran hastalarda maliyet artmıştı. Ortalama yatış süresi 36.3±18.1 gündü ve beklenildiği gibi yatış süresi uzadıkça maliyet artıyordu. Toplam yatış maliyeti ve rehabilitasyon maliyeti trombotik nedenli hastalarda embolik ve kanama nedenli olanlara göre daha yüksekti. Toplam yatış maliyeti ve komplikasyon maliyeti ise brunnstrum evre 3 hastalarda diğer evrelere göre daha yüksekti. Toplam yatış maliyeti paralel bar içinde terapötik amaçlı ambule hastalarda daha yüksekti. Taburcukta yatak ve tekerlekli iskemle düzeyindeki hastalar ile bağımsız hastaların maliyetleri diğer hastalara göre daha düşüktü.
ABSTRACT
The economic cost of inpatients stroke patients in rehabilitation unit was evaluated in this study. Four hundred and sixty-five stroke patients who admitted to rehabilitation center between 2000 and 2004 were assessed. Patients files and their invoice at discharge were used to describe the economic cost in patients management. Cost per stay, cost per day were determined for all patients. Cost per stay were divided in to subgroups; rehabilitation cost (all rehabilitative, medical and complication management and their cost), laboratory cost and hospital cost. Age and sex had no effect on cost. Cost per stay was increased for patients admitted later to the rehabilitation unit. The mean lenght of stay was 36.3±18.1 days and as expected increasing lenght of stay caused increase in cost per stay. Cost per stay and rehabilitation cost were higher in patients with thrombotic stroke compared with embolic and hemorrhagic ones. Cost per stay and complication cost were higher in patient with brunnstrum stage 3 compared with other stages. Cost per stay was higher in ambulated patients at the level of paralel bar with therapeutic purpose. Patients at the level of bed and wheelchair and independent patients at discharge had lower cost per stay compared with others.
