ÖZET
Dengenin sağlanmasında vestibüler, oküler ve proprioseptif sistem birlikte rol oynamaktadır. Stabil dik postür ve baş ve göz hareketlerinin sensorimotor kontrolü santral sinir sisteminde birbirine yakın alanlar üzerinde vestibüler, vizüel ve proprioseptif sistemlerden gelen afferent “input”larla sağlanmaktadır. Servikal omurganın proprioseptif “impuls”un sağlanmasında önemli rolü vardır. Servikal intervertebral eklemlerin mekanoreseptörleri, boyun kasları ve ligamanların mekanoreseptörleri, servikal omurganın derin kaslarında lokalize kas iğcikleri ve spinal kordun kornu posteriorundaki nöronlarla boyun proprioseptörlerini birleştiren sensitiv liflerden oluşan servikal proprioseptif sistemin dengenin sürdürülmesinde önemli bir yeri vardır. Boyun hastalıklarında proprioseptör reseptör disfonksiyonuna bağlı olarak sensorimotor bozukluklar yaygındır. Boyun hastalıklarında servikal reseptör disfonksiyonu sensorimotor kontrolün birleştirilmesi, zamanlaması ve dönüştürülmesini etkileyerek afferent “input”ta değişikliklere yol açabilmektedir. Boyun hastalığı olanlarda “dizziness”, değişmiş servikal propriosepsiyon, baş-göz koordinasyonu ve postüral stabilitedeki bozuklukların değerlendirilmesi ve tedavisi önemlidir. Son çalışmalar, boyun hastalıklarında sensorimotor kontrol bozukluklarına dikkat çekmektedir. Servikal eklem pozisyon duyusu, okülomotor kontrol ve postüral stabilitede ölçülebilir değişikliklerin değerlendirilmesi sensorimotor kontroldeki bazı değişiklikler ile ilgili olabilir. Bu nedenle boyun ağrılı hastaların tedavisi planlanmadan önce sensorimotor bozuklar değerlendirilmeli ve dikkate alınmalıdır.
ABSTRACT
Vestibular, ocular and proprioceptive systems acts together while achieving the proper balance. Sensorimotor control of stable upright posture and head and eye movement relies on afferent information from the vestibular, visual and proprioceptive systems, which converge in several areas throughout the central nervous system. The cervical spine has an important role in providing the proprioceptive input. Cervical proprioceptive system consisting of mechanoreceptors of cervical intervertebral joints, mechanoreceptors of neck muscles and ligament's insertions, muscle spindles located in deep short muscles of cervical spine and sensitive fibers connecting neck's proprioceptors with neurons of cornu posteriori of spinal cord, plays an essential part in maintaining bodily balance. Sensorimotor disturbances in neck disorders are common because of the proprioceptive receptor dysfunction. Dysfunction of the cervical receptors in neck disorders can alter afferent input subsequently changing the integration, timing and tuning of sensorimotor control. Assessment and management of the symptoms of dizziness, altered cervical proprioception, eye and head co-ordination and disturbances to postural stability in those with neck disorders are important. Recent research highlights sensorimotor control disturbances in those with neck disorders. Assessment of measurable changes in cervical joint position sense, oculomotor control and postural stability can be related to such alterations to sensorimotor control. Therefore, sensorimotor disturbances must be evaluated and should be taken into consideration before planning the treatment of patients with neck disorders.
