Spinal Kord Yaralanmalı Bireylerde Nörojenik Bağırsak Disfonksiyonu ile Beslenme Arasındaki İlişki
The Relationship Between Neurogenic Bowel Dysfunction and Nutrition in Individuals with Spinal Cord Injury

ÖZET
Amaç: Çalışmamızda, spinal kord yaralanmalı (SKY) bireylerde, nörojenik bağırsak disfonksiyonu (NBD) ile beslenme arasındaki ilişkiyi değerlendirmeyi amaçladık. Gereç ve Yöntemler: On sekiz yaş üstü ve olay süresi 6 aydan uzun olan 50 travmatik veya nontravmatik SKY’li birey çalışmaya dâhil edildi. Hastaların NBD ciddiyetini belirlemek için NBD skoru ve besin ögesi alım miktarlarını belirlemek üzere Beslenme Bilgi Sistemi-BeBis 7.2 versiyonu kullanıldı. Bulgular: Hastaların ortalama yaralanma süresi 39±36,4 aydı ve 31 (%62) hasta motor komplet yaralanmaya sahipti. Otuz sekiz (%76) hasta refleks, 12 (%24) hasta arefleks NBD olarak değerlendirildi. Hastaların ortalama NBD skoru 12,6±7,5 ve haftalık bağırsak boşaltım sıklığı 4,3±2,8 (1-14) olarak saptandı. Motor komplet hastaların NBD skorları, motor inkomplet hastalara göre anlamlı olarak yüksek bulundu (p=0,012). Hastaların posa alım miktarı 19,5±8,0 g/gün olarak saptandı. Posa alımı ile haftalık bağırsak boşaltım frekansı arasında hem refleks (p=0,030, r=0,353) hem arefleks NBD’si (p=0,001, r=0,834) olan hastalarda pozitif korelasyon saptandı. Refleks NBD’si bulunan hastalarda, posa alımı ile NBD skoru arasında anlamlı negatif korelasyon saptanırken (p=0,039, r=-0,336); arefleks NBD’si olan hastalarda, posa alımı ile NBD skoru arasında anlamlı korelasyon saptanmadı (p=0,414, r=-0,260). Sonuç: Diyette yüksek posa alımının; SKY'li bireylerde bağırsak boşaltım frekansını arttırdığı ancak bunun NBD skorlarındaki olumlu etkisinin sadece refleks NBD'si bulunan SKY'li bireylerde görüldüğü saptandı.
ABSTRACT
Objective: In our study, we aimed to evaluate the relationship between neurogenic bowel dysfunction (NBD) and nutrition in individuals with spinal cord injury (SCI). Material and Methods: Fifty traumatic or non-traumatic SCI individuals over the age of 18 and with an injury duration >6 months were included in the study. NBD score was used to determine patients' NBD severity. BeBis version 7.2 was used to determine nutrient intakes of the patients. Results: The mean duration of injury was 39±36.4 months and 31 (62%) patients had motor complete injury. Thirty eight (76%) patients were evaluated as reflex, 12 (24%) patients were evaluated as areflex NBD. The mean NBD score of the patients was 12.6±7.5 and the weekly bowel movement frequency of the patients was 4.3±2.8 (1-14). The NBD scores of the patients with motor complete injury were found to be significantly higher than those of the patients with motor incomplete injury (p=0.012). The mean fiber intake of the patients was 19.5±8.0 g/day. A positive correlation was found between fiber intake and bowel movement frequency in patients with both reflex (p=0.030, r=0.353) and areflex NBD (p=0.001, r=0.834). In patients with reflex NBD, a significant negative correlation was found between fiber intake and NBD score (p=0.039, r=-0.336), no significant correlation was found between fiber intake and NBD score in patients with areflex NBD (p=0.414, r=- 0.260). Conclusion: It was found high dietary fiber intake increased the frequency of bowel evacuation in individuals with SCI, but this had a positive effect on NBD scores only in those with reflex NBD.
