ÖZET
Amaç: Karpal tünel sendromu (KTS) tanısı alan hastalarda
semptom, bulgu ve işlevsellik düzeylerinin kompresyon seviyeleri ile
ilişkisinin ortaya konulmasıdır. Gereç ve Yöntemler: 2018-2019 Nisan
tari arasında Medipol Üniversitesi Sefaköy Sum Hastanesi Fizik Tedavi
ve Rehabilitasyon ve Nöroloji Polikliniklerine medyan sinir innervasyon
bölgesinde uyuşma ve ağrı şikâyetleri ile başvuran ve
elektronöromiyografi (ENMG) inceleme yöntemi ile KTS tanısı alan
74 (132 el bileği) hasta çalışmaya alındı. Hastaların demografik bilgileri
sorgulandı. Semptom şiddetini değerlendirmek için Boston Semptom
Ciddiyet Ölçeği (BSCÖ) ve vizüel analog skala (VAS), motor
kuvveti değerlendirmek için Jamar el dinamometresi ile el ve parmak
kavrama kuvveti ölçümü, fonksiyonel durumu sorgulamak için Duruöz
El Skalası (DES) ve Fonksiyonel Durum Ölçeği (FDÖ) uygulandı. Bulgular:
Çalışmaya grubunun ortalama yaşı 49,7±12,13 yıl ve %87,84’ü
kadındı. Hastaların %78,38’inde bilateral, %17,57’sinde sağ taraf tutulumu
mevcuttu. Yapılan ENMG incelemelerinin %46,2’sinde Evre
1, %37,1’inde Evre 2, %16,7’sinde Evre 3 KTS saptandı. Hastaların
ENMG sonuçları ile klinik bulgularını ve fonksiyonel durumunu değerlendiren
testler karşılaştırıldığında, medyan sinir kompresyon şiddeti
ile ortalama VAS skorları arasında anlamlı ilişki saptanırken (p<0,001);
ortalama BSCÖ skorları (p=0,051), el-parmak kavrama kuvvetleri, ortalama
DES skorları (p=0,439), ortalama FDÖ skorları (p=0,195) açısından
gruplar arasında anlamlı fark yoktu. Sonuç: Çalışmamızda,
medyan sinir kompresyon şiddeti ile sadece VAS arasında ilişki saptanmış
olması, daha geniş kapsamlı çalışmalara ihtiyaç olduğunu göstermekteyken;
klinik bulguları değerlendiren testler ve ölçeklerle ilgili
çelişkili sonuçlar, özellikle evreleme ve tedavi kararını belirlemek konusunda
elektrofizyolojik çalışmaların gerekliliğini bir kez daha ortaya
koymaktadır.

ABSTRACT
Objective: The aim of this study was to investigate the
relationship between severity of impingement and clinical symptoms,
physical findings and functionality in patients diagnosed with carpal
tunnel syndrome (CTS). Material and Methods: Between April 2018-
2019, 74 (132 hand wrist) patients admitted to Medipol University Sefaköy
Sum Hospital Physical Therapy and Rehabilitation and
Neurology Outpatient Clinics with numbness and pain in hand and diagnosed
as CTS by electroneuromyography (ENMG) were included.
Demographic characteristics of the patients were questioned. Boston
Symptom Severity Scale (BSSS) and visual analogue scale (VAS)
were used to assess symptom severity, hand and finger grip strength
measurements were performed to assess motor strength, and Duruöz
Hand Scale (DHS) and Functional Status Scale (FSS) were used to determine
functional status. Results: The mean age of the study group
was 49.7±12.13 and 87.84% of them were female. 78.38% of the patients
had bilateral and 17.57% had right side involvement. Stage 1,
Stage 2, Stage 2, and Stage 3 CTS were detected in 46.2%, 37.1% and
16.7% of the ENMG examinations. When the ENMG results of the
patients and the tests evaluating their clinical findings and functional
status were compared, a significant correlation was found between the
median nerve compression severity and mean VAS scores (p<0.001),
there was no significant difference between mean BSSS scores, handfinger
grip strength, mean DHS scores, and mean FSS scores in the
groups. Conclusion: In our study, the relationship between the severity
of median nerve compression and VAS alone showed that larger
studies were needed. The contradictory results of the tests and scales
evaluating the clinical findings once again reveal the necessity of electrophysiological
studies to determine the staging and treatment decision.
