GENÇ ALT EKSTREMİTE AMPUTASYONLU HASTALARDA LOKOMOTOR KAPASİTE İNDEKSİ VE HOUGHTON SKALA GÜVENİLİRLİĞİ

ÖZET
Amaç: Bu çalışmanın amacı genç alt ekstremite amputasyonlu hastalarda Lokomotor Kapasite İndeksi (LKİ) ve Houghton Skalanın (HS) güvenilirliğini değerlendirmektir.

Materyal ve Metod: Çalışmaya alt ekstremite amputasyonu olan 99 genç hasta [21 ayak bilek (AB), 61 transtibial (TT), 17 diz üstü (DÜ)] alındı. LKİ ve HS içsel tutarlılık, test-tekrar test güvenilirlik (1 ay sonra) ve tavan etkisini araştırmak için uygulandı.

Bulgular: LKİ için Cronbach's alpha=0.90, HS için 0.67 bulundu. Test-tekrar test korelasyon katsayısı LKİ total skoru için 0.93, HS total skoru için 0.90 olarak hesaplandı. LKİ için AB amputasyonlu hastaların tamamı, TT amputelerin % 98'i, DÜ amputelerin % 41'i tam puan aldı. HS için AB amputasyonlu hastaların % 43'ü, TT amputelerin % 53'ü, DÜ amputelerin % 6'sı tam puan aldı.

Sonuç: LKİ ve HS'nin Türkçe versiyonunun içsel tutarlılığı ve test-tekrar test güvenilirliği yüksek bulunmasına rağmen özellikle AB ve TT amputasyonu olanlarda gözlenen yüksek orandaki tavan etkisi nedeniyle genç amputelerde araştırma amaçlı kullanılmasının uygun olmadığı düşünüldü. Genç amputelerin mobilite gelişimini takip için LKİ'in psikometrik özellikleri geliştirilmiş yeni versiyonları üzerine yeni çalışmalar yapılması gerekmektedir.

ABSTRACT
Aim: The aim of this study was to assess the reliability of the Locomotor Capabilities Index (LCI) and Houghton Scale (HS) among young lower limb amputees (LLA).

Materials and Methods: Ninety-nine young patients with LLA [21 ankle foot (AF), 61 transtibial (TT), 17 upper knee (UK)] were included in the study. The standard LCI and HS were tested for internal consistency, test-retest reliability (after 1 months) and the ceiling effect.

Results: The Cronbach's alpha of the LCI was 0.90 and of the HS was 0.67. Test-retest correlation coefficient for the total scores was 0.93 for LCI and 0.90 for HS. Ceiling effects of the LKI was 100% for the AF and TT amputees and 41% for UK amputees. Ceiling effects of the HS was 43% for the AF, 53% for the TT, 6% for the UK amputees.

Conclusion: Although test-retest reliability and internal consistency of the Turkish version of LKI and HS are acceptable, they can not be recommended to research use because of high ceiling effect in young patients with LLA. Further testing is necessary to study the psychometric properties of the new version of LKI.
