ÖZET
Amaç: Botilinum Toksin A'nın hemiplejik spastik serebral palsi hastalarında üst ekstremite fonksiyonlarına etkisinin değerlendirilmesi.

Metod: Hemiplejik spastik serebral palsi tanısı konulan ortalama 4.9 yaşında 9 hastanın ön kol, el bileği ve elde spastisiteye bağlı gelişen fonksiyonel yetersizlikleri, Modifiye Ashworth Skalası, Zancolli sınıflaması ve Üst Ekstremite Doktor Puanlama Skorlaması kullanılarak değerlendirildi. Botulinum Toksin A enjeksiyonları (Botox® 1-2 Ü/kg, Dysport® 3-6 Ü/kg) ön kol pronator, fleksör ve başparmak kaslarına yapıldı ve toplam 21 seans fizyoterapi uygulandı. Enjeksiyon öncesi değerlendirme sonuçları, 6,12 ve 24. haftalardaki sonuçlar ile karşılaştırılarak uygulamanın etkinliği incelendi.

Bulgular: Modifiye Ashworth Skalası kullanılarak yapılan spastisite derecelendirmesine göre, 6, 12 ve 24. haftalarda spastisite kademeli olarak artmakla beraber Zancolli sınıflaması ve Üst Ekstremite Doktor Puanlama Skorlamasına göre kazanılan fonksiyonların daha uzun süre korunabildiği saptanmıştır.

Sonuçlar: Uygun seçim yapılmış hemiplejik spastik hastalarda Botilinum Toksin A enjeksiyonu ile üst ekstremite spastisitesinin giderilmesi ve fizik tedavi programı ile desteklendiğinde bazı motor fonksiyonlarda kazanımların sağlanması mümkün olabilecektir.

ABSTRACT
Aim: To evaluate the effects of Botulinum Toxin A on the upper extremity functions in hemiplegic spastic cerebral palsy.

Methods: Nine patients with the average age of 4.9 were diagnosed as hemiplegic spastic cerebral palsy and functional impairments on their forearm, wrist and hand due to muscular spasticity were evaluated with the Modified Ashworth Scale, Zancolli classification, and Upper Limb Physician's Rating Scale. Botulinum Toxin A injections (Botox® 1-2 Ü/kg, Dysport® 3-6 Ü/kg) were made in the forearm pronator, flexor and thumb muscles and totally 21 sessions of phsyiotherapy were carried out. The pre-injection evaluation results were compared with the results at the 6th, 12th and 24th weeks.

Results: Although the muscular spasticity gradually increased according to the Modified Ashworth Scale at the 6th, 12th and 24th weeks after the Botulinum Toxin A injection, Zancolli classification and Upper Limb Physician's Rating Scale results showed that the gained functions were preserved for a longer time.

Conclusion: In appropriately selected hemiplegic spastic patients Botulinum Toxin A injection decreases the upper extremity spasticity and when supported with physical rehabilitation program it might be possible to gain some motor functions.
