The Effect of Neuromuscular Electrical Stimulation on Muscle Strength and Functionality in Rheumatoid Arthritis

ÖZET
Amaç: Bu çalışma romatoid artrit (RA) hastalarında kuadriseps kas atrofisinde nöromuskuler elektrik stimulasyonu uygulamasının hastalık aktivitesine, günlük yaşam aktivitelerine ve kas kuvvetlerine etkisini değerlendirmek için planlandı.

Yöntemler: Çalışmaya 33 RA hastası dahil edildi. Hastalar elektrik stimulasyonu grubu (n=11), elektrik stimulasyonu + egzersiz grubu (n=12) ve egzersiz grubu (n=10) olmak üzere üç gruba ayrıldılar. Elektrik stimulasyonu ve elektrik stimulasyonu + egzersiz grubunda yer alan hastalara 4 hafta haftada 3 kez Rus akımı ile kuadriseps kası stimülasyonu uygulandı. Egzersiz grubuna ise dize yönelik egzersiz eğitimi verildikten sonra evde günde bir defa olmak üzere gösterildiği şekilde hareketleri yapmaları önerildi. Tedavi programları başlanmadan önce ve bitiminde hastalık aktivite skorları (DAS28), günlük yaşam aktivite değerlendirmeleri ve izokinetik dinamometre ile döndürme momenti tepe değerleri 60 ve 180°/sn açısal hızda değerlendirildi.

Bulgular: Tedavi öncesi ve sonrasında hastalık aktivite skorlarında ve fonksiyonelliği değerlendiren parametrelerde istatistiksel anlamlı fark saptanmazken (p≥0,05), diz ekstansiyonu döndürme momenti tepe değerinde 60°/sn açısal hızda sağda (p=0,002), 180°/sn açısal hızda sağda ve solda anlamlı fark (p=0,026, 0.022) saptandı. kuvvet artışları nöromüskuler elektrik stimülasyonu ile egzersiz tedavisinin birlikte verildiği grupta en fazla saptandı.

Sonuçlar: Nöromüskuler elektrik stimulasyonu ve egzersiz programının birlikte verilmesi egzersiz programlarına katılamayan RA hastalarında kuadriseps kas atrofisinin tedavisinde uygulanabilecek bir yöntemdir. (FTR Bil Der 2011;14: 68-74)

ABSTRACT
Objective: This study was designed to evaluate the effect of neuromuscular electrical stimulation on disease activity, daily life activities and muscle strength in rheumatoid arthritis patients.

Methods: Thirty three rheumatoid arthritis patients were divided into three groups; the first group received only the electrical stimulation therapy (n=11), the second group received both the electrical stimulation and the exercise therapy (n=12) and the third group received only the exercise therapy (n=10). The groups treated with electrical stimulation and with electrical stimulation plus exercise were given Russian type of current 3 times in a week for four weeks for the stimulation of the quadriceps muscle. The patients in the exercise group were trained to do knee exercises at home once a day. Before the beginning and after the end of the treatment sessions, disease activity scores(DAS 28), daily life activities and the peak values of the peak torques at angular velocities of 60 and 180°/sec. were assessed.

Results: Before and after the treatment sessions, no statistically significant difference found in disease activity scores and parameters that evaluate the functionality (p≥0.05). However there is a statistically significant difference in peak torques of knee extension at the right side at angular velocity of 60°/sec (p=0.002), and at both sides at angular velocity of 180°/sec (p=0.026, 0.022). The improvement of muscle strength was the highest in the group which received exercise together with electrical stimulation.

Conclusion: Electrical stimulation in combination with exercise therapy should be considered in rehabilitation of rheumatoid arthritis patients. (J PMR Sci 2011;14: 68-74)
