ÖZET
Bu çalışmada 1994-1997 yılları arasında kliniğimize başvuran ve cerrahi olarak anteriordan füzyonlu ve füzyonsuz diskektomi ile posterior laminektomi uygulanan servikal spondilotik miyelopatili 32 hasta incelenmiştir. Hastaların tanısında Manyetik Rezonans Görüntüleme (MRG) önemli bir yer oluşturmaktadır ve 29 olgu (%91) ile büyük çoğunluğunda ilk tanı yöntemi olmuştur. Servikal spondilotik miyelopatili olguların 14'üne füzyonsuz, 10'una füzyonlu anterior yolla diskektomi ve 8'ine de posterior laminektomi uygulandı. Anterior yaklaşım yapılan olgularda füzyonlu ve füzyonsuz anterior diskektomiler en çok tek mesafelere uygulanmıştır. Posterior yaklaşımla total laminektomi ise en çok üç mesafe yapılmıştır. Sonuçlarımız; Japonya Ortopedi Birliği (JOA) nin servikal miyelopati değerlendirme skorlarına göre iyileşme oranları hesaplanarak değerlendirildi. Füzyonsuz anterior servikal diskektomide mükemmel sonuç görülmezken, iyi sonuç 4 olgu (%29), orta 8 olgu (%57) ve kötü 2 olgu (%14) olduğu görüldü. Füzyonlu anterior servikal diskektomide mükemmel sonuç 2 olgu (%20), iyi 6 olgu (%60) ve orta 2 olgu (%20) olarak saptanırken kötü sonuç görülmedi. Posterior laminektomi yapılan grupta ise mükemmel ve iyi sonuç görülmezken, orta 6 olgu (%75) ve kötü sonuç 2 olgu (%25) olarak saptandı. Anterior ve posteriordan yaklaşımın seçimi ile, anteriordan füzyon yapılıp yapılmaması ile ilgili tartışmalara katkıda bulunmak amacıyla, sonuçlarımız hangi prosedürün daha üstün olduğunun saptanması için literatür ışığında tartışıldı.
ABSTRACT
In the present study, 32 patients consecutively treated between 1994-1997 with anterior discectomy with and without fusion and posterior laminectomy for cervical spondylotic myelopathy were retrospectively reviewed. First choice for diagnosis was Magnetic Rezonance İmaging (MRI) in 29 cases (91%). Anterior cervical discectomy without fusion was performed in 14 cases, the same procedure with fusion in 10 cases and posterior laminectomy in 8 patients. The most patients who were operated by anterior procedure were one level spondylosis. Posterior laminectomy was performed maximum in three levels. Clinical outcomes were graded from excellent to poor using Japan Orthopaedic Association (JOA)'s criteria. In the anterior cervical discectomy without fusion group, the results were good in 4 cases (29%), satisfactory in 8 cases (57%) and poor in 2 cases (20%) where no any excellent results in this group. In the anterior discectomy with fusion group, the results were excellent in 2 cases (20%), good in 6 cases (60%) and satisfactory in 2 cases (20%) where no any poor results was seen. In the posterior laminectomy group, there were satisfactory in 6 cases (75%) and poor results in 2 cases (25%) where no any excellent and good results were seen in this group. In this manuscript, the results of different surgical procedures such as anterior versus posterior intervention and with or without fusion techniques in cervical spondylotic myelopathy cases were discussed in the view of literature.
